Required Paperwork

Below is the required paperwork for each student reporting to Officer Training Command Newport, including prior service members. All paperwork will be returned to you at the end of the course.

*Not having all required paperwork will delay your entry into Active Duty and the ability to receive pay.*

Even if you are currently receiving pay, it is strongly recommended to bring all documentation in case of administrative issues—it’s better to have it and not need it than to face delays.

All Active Duty/SELRES/Graduated HPSP:

(Legible copies of all documents are acceptable unless otherwise noted.)

  • Two (2) sets of your Orders
  • Oath of Office (must be signed in ink by a witnessing Officer)
  • Recruiter Contact Information (Name, Phone, Fax, and NRD Address)
  • Bank Direct Deposit form (or blank/voided check if unavailable)
  • Birth Certificate (Original or certified copy)
  • Social Security Card
  • Original, unexpired state or government-issued photo identification
  • All completed paperwork from recruiter (bring copies even if records are sent to ODS)
  • W-4 form
  • Servicemembers Group Life Insurance (SGLI) Certificate of Coverage
  • DD Form 2058 - State of Legal Residence
  • DD Form 93 - Record of Emergency Data
  • Marriage License (if applicable, Original)
  • Divorce documents (if applicable)
If Receiving an Accession Bonus:
  • Service Agreement
Additional Documentation for Students with Dependents

You will need to wait until you have been strength gained into the Navy and receive a CAC to enroll your dependents in the Defense Enrollment Eligibility System (DEERS). DEERS requires original or court-certified copies to enroll your dependents into DEERS.

  • Spouse’s original unexpired identification card (driver's license, state ID, or passport)
  • Birth Certificates for all dependents (Original or certified copy)
  • Social Security Cards for all dependents
  • Naturalization certificate or U.S. birth certificate for dependents born outside the U.S.
  • Foreign documents must be translated into English by an official translator or translation service if not already in English
  • Child custody documents (if applicable)
Additional Documentation for Students with Prior Military Service
  • All documents reflecting prior service, such as DD-214, DD-368, NGB-22, and Reserve Points Summary
  • Ensure orders are fully read, including faxing any DD-214 to relevant entities (e.g., PERS-831 or NPC) per instructions
  • Stamped orders from prior/detaching command
  • Page 2 (Record of Emergency Data) verified through NSIPS, required for Basic Allowance for Housing (BAH) payment
  • Contact information for your last personnel office (PSD, S-1, IPAC, or equivalent)
  • SGLI Certificate from Milconnect
For Current (Non-Graduated) HPSP Students Only:
  • PCS or Reserve Orders and any modifications
  • Verify Page 2 ("Record of Emergency Data") is up-to-date within the last 12 months; update with the HPSP office if needed
  • Contact HPSP office in Bethesda, MD, for pay issues (email USN.OHSTUDENT@MAIL.MIL for assistance)
  • It is recommended to bring a copy of immunization records
  • For pay and administrative issues, contact the CCPO program manager directly

Action Items Prior to Arrival at ODS

  • Inform financial institutions of your limited access to dual authentication during training. Consider setting accounts to single authentication if possible.
  • Ensure all personal financial obligations are managed to avoid the need for ongoing attention during training.
  • Consider granting Limited Power of Attorney to a trusted individual for managing any unanticipated issues (e.g., loan payments, DEERS access).
  • Arrive within Navy grooming standards. Refer to Navy Grooming Standards for details.

Medical Documentation

Medical Documentation Required:
Any medical paperwork you turn in on Monday will not be returned. Make copies prior to arrival if you wish to retain additional copies.

  • Copy of any prescriptions and any other medical documentation supporting the need for a prescription.
  • A 5-week supply of all prescriptions should be brought to ODS in the original container.
  • Bring all recent medical and dental records from both military and civilian treatment facilities.
Immunizations and Labs

Bring proof of all immunizations received throughout your lifetime. The following immunizations will be administered at ODS if no proof is provided:

  • TWINRIX (Hep A/Hep B)
  • HEP A Series
  • HEP B Series
  • Tetanus (Tdap)
  • IPV (Polio)
  • Seasonal Influenza
  • PPD (Tuberculosis)
  • Varicella
  • MMR (Measles, Mumps, Rubella)
The Following Labs Will Be Drawn at ODS:
  • HIV
  • Hep B Titer
  • Varicella Titer
  • ABO/RH (Blood type)
  • IPV (Polio)
  • V Zoster IGG
  • DNA
  • G6PD/Sickle Cell

** Note: Laboratory bloodwork must be completed at ODS and cannot be accepted from an outside facility.
